  2.  Request a VA Community Care referral for acupuncture.
     
  3.  Request Dr. Nadia Ayadi at Eastlake Acupuncture as your preferred provider and provide the following National Provider Identifier (NPI) for easy lookup: 1265790604.
     
  4.  Once approved, acupuncture treatments are fully covered by the VA at no cost to you. 


Once your referral is authorized, I will assist you with scheduling your appointments and guide you through your first visit.
